PROCEDURE FOR EFFECTING CHANGE OF NAME IN THE INSTITUTES RECORDS
Dear Member,
The change in the name in the record of the Institutes will be noted under the following circumstances:
a. Duly signed application form (form available at Institute’s website : https://www.icsi.edu/member/formsformembers/
b. Self-attested copy of Marriage Registration Certificate OR Marriage invitation card in original OR an affidavit (in original) on non-judicial stamp paper of Rs.10/- affirming the date of marriage with details duly attested by First Class Magistrate/Notary Public/Oath Commissioner as the case may be
c. Copy of PAN Card (with Old Name)
d. Copy of PAN Card (with new name)
e. PAN Database (retrievable from the Income Tax website)
f. Any ID proof i.e. AADHAAR card OR copy of PASSPORT with new name. (one copy of Old name and one copy of new name)
2. IN ALL OTHER CASES: Application for change in name along with self-attested copy of Extract of Gazette notification and original advertisement as published in a leading newspaper.
All above documents should be self-attested.
